The of the West Stockbridge Police Web Site is to make the Department more accessible to the public through the use of modern technology. Part of this effort includes making the forms many people need available for downloading free of charge over the internet. If you are involved in a motor vehicle accident where someone is injured, and/or if there is over $1000 in damage to a vehicle or property, you must submit an accident report to the Police Department and MA Registry of Motor Vehicles within five days.
This form is used to file a complaint with the MA Registry of Motor Vehicles if you witness the improper operation of a motor vehicle.
Use this form to notify the Firearms Record Bureau that your address has changed.
Use this form to apply for a Permit to Carry firearms. Be sure to view and follow the instructions completely.
Use this form to apply for a FID Card. Be sure to view and follow the instructions completely.
Use this form if you would like a copy of your own Massachusetts criminal record.
Parents may receive conviction and pending case data on a current or prospective child care provider who has the potential for direct and unmonitored contact with their children, and who are otherwise qualified for the position.
This form is made available to anyone requesting sex offender information from any city/town police departments.
